Register — Oxbow Dance

I understand that participation in dance classes, rehearsals, performances, workshops, and other activities offered by Oxbow Dance involves physical activity and carries inherent risks, including but not limited to slips, falls, collisions, sprains, strains, fractures, cuts, and other injuries. I voluntarily allow my child to participate in these activities and acknowledge that I understand and accept these risks.In consideration of my child's participation in Oxbow Dance activities, I hereby release, waive, discharge, and hold harmless Oxbow Dance, its owners, directors, instructors, teachers, assistants, volunteers, guest instructors, choreographers, staff members, and any facility used for dance activities from any and all claims, demands, actions, causes of action, damages, losses, costs, or expenses arising from or related to any injury, illness, accident, loss, or damage sustained by my child while participating in Oxbow Dance activities, except where such injury results from gross negligence or willful misconduct. I understand and agree that Oxbow Dance is not responsible for any injuries that may occur during classes, rehearsals, performances, or any related activities.In the event of an emergency, I authorize Oxbow Dance staff, instructors, assistants, or volunteers to seek appropriate medical treatment for my child if I cannot be reached immediately. I understand that I am solely responsible for any medical expenses incurred.I understand that Oxbow Dance is not responsible for lost, stolen, or damaged personal belongings brought to classes, rehearsals, or performances.
